It is free to participate in the Beloved Memories Project. One complimentary game (includes box and 32 personalized memory cards) will be printed and given to theAlzheimer's participant in hopes to preserve that person's life for generations to come. Click here to download the PDF packet. You will find a Participant Application, Art Release Form, and Memory Sheet. Please fill out each form in its entirety. Once you are ready, please contact email me to schedule your one-on-one interview sessions. We conveniently schedule interviews at the participant's preferred location, either at their home, assisted living facility, or even a local restaurant.
